Lillian was married to Beth's father. If I remember correctly, he was killed in a car accident when Beth was young. She later married Bradley Rainses, of which Beth took his last name. I am not sure if he truely adopted her as he was always referred to as step dad. This was all off screen. Beth was roomates with Mindy in the hospital and Rick and Phillip, got to meet Beth first then. Lillian never married after Brad until just this week when she married Buzz.
