"It should be an easy shot," Ian tells Jerry as they set up the sniper attack against Sonny. He tells Jerry that his percentage of the drug sales could be doubled if only he would do the job instead. Jerry is happy with his cut as it is and doesn't want to risk his relationship with his brother.
"Since when do you balk at murder?" Ian asks Jerry during their waterfront meeting. Before he can answer, Alexis walks up and asks the two men how they know each other. Jerry says he was simply inquiring about her nephew's condition and Dr. Devlin, grateful for the lie, leaves. Jerry tells her that he's a bit worried that the drugs he gave Nikolas might have something to do with his illness and if that's true, he'll never have a chance to romance her. She tells him that she's never heard one word of regret for his awful actions and that would be a start. He reminds her that you can't unring a bell, however he is trying to reprioritize his life for the right reasons. She tells him that she is not rescuing another bad boy. He kisses her in the middle of her proclamation and then tells her that she liked that as much as he did. She can't deny it, but says that she's only human.
Claudia has Jerry and Ian meet her at Sonny's warehouse. She tells them that Sonny will be there on Friday at 7:00 p.m. Ian looks around and says that he can handle it and Claudia seems pleased.
Claudia meets Dr. Devlin on the waterfront and says that Sonny is in their way. Jerry shows up and Dr. Devlin introduces him as his partner. Claudia says that she doesn't work with strangers. "If Sonny Corinthos is to die, Jerry is the perfect man to kill him," Ian tells her. Claudia tells Jerry that it can't be traced back to her family in any way or a mob war will break out. After she leaves, Jerry tells Ian never to make plans for him without his knowledge. He says that he will not kill Sonny because he is the father of his brother's step children and that nothing is worth losing a brother. He suggests that Ian pull the trigger himself.
After Nik leaves Monica, Dr. Devlin finds him in the hallway. He says that he heard about Nik's new program to help patients without insurance and about his plans to run it personally. He comments about how easy it will be for him to slip Nik the meds now. Nikolas says that he expects the meds no matter how hard it is for Devlin to deliver them. Nadine approaches Nikolas after Devlin walks on and asks why he is planning a new program if he's not going to live long enough to see it into fruition?
At the Zacchara mansion, Claudia asks Devlin how he can deal drugs and not be conflicted. He says to let him worry about that and to worry about Trevor finding out what is going on. Claudia says Trevor doesn't matter because she and Johnny are in control. Devlin says there will be a shipment in a month; Claudia assures him that Trevor is on the way out. A while later Johnny arrives and she tells him they need to take over Alcazar's piers for the drug deal. As they talk, Trevor listens. After a while he comes inside and says he is still in control of the family. He threatens to send Johnny back to another family member to better learn the business and then tells him not to believe Claudia because she has no clue how to run the family.
Nikolas arrives at the hospital and takes Devlin aside. He tells Devlin to get him a supply of the drug - or else! Devlin reminds Nik that the drug isn't legal and he'll raise suspicion if he keeps coming to the hospital. Claudia and Johnny arrive, make up a story about Claudia's stitches and ask to see him.
At home, Nikolas struggles with what treatment option to take. Emily tells him that he has to go all the way and have the tumor removed; Nik doesn't want that because it means he will lose her. Devlin arrives and is uncomfortable that someone may have recognized him. Nik says not to worry and asks for more medication. Devlin reminds Nikolas that the medication isn't a cure and that it could backfire. Then, he says he wants $10 million to continue the treatments! Nik laughs but then goes along with the deal. Devlin leaves. Emily returns and tells Nikolas that he shouldn't be working with Devlin. He kisses her and says he can handle the doctor.
At the Zacchara mansion, Claudia tells Johnny that he needs to be very careful of Sonny - and back off the talk of war! Johnny pulls his head-of-the-family card and tells Claudia she can't stop him from taking care of Sonny. He says she shouldn't worry because even if he is killed she will be there to take over. Claudia insists they need to be sneakier about their attack on Sonny but Johnny won't listen. Claudia keeps pushing. "We're talking about an all-out mob war that will cost us money - and lives," she says and then suggests they get a third party to kill Sonny for them and reason with Jason after the fact. They are still arguing when Sonny's arrival is announced. Johnny grabs a gun but Claudia gets him to put it away before Sonny walks in. Sonny tells Johnny that he made a mistake and that he is there to negotiate a truce! He says he'll leave them alone if they'll leave him alone, he won't go after Alcazar's piers if they won't and that they have to keep drugs out of his areas. Claudia doesn't like the deal but Johnny agrees to go along! Sonny leaves. Claudia turns to Johnny who says he is still going after Corinthos! He tells her to find that third party and go ahead with knocking off Sonny. Claudia is alone in her office when Devlin arrives and says he has a large drug shipment coming in and a new source of income - and he wants the Zacchara's to deliver his drugs. Claudia says they can handle it.
Patrick is working on papers at the admissions desk with Epiphany when Devlin walks up. Devlin brings up Robin and says Patrick is lucky to not be a father. Elizabeth returns to work and asks Patrick about a patient. Seeing how distracted he is, Liz asks what is wrong. He tells her about Carly and Jax, relating that to his situation with Robin. "I didn't want to be a father," he says, "but if Robin lost the baby I would feel that loss."
