At the hospital, Edward brings Tracy coffee, but she's acting difficult as is her way when she's hurting. Edward talks about all the things that he did wrong when it comes to Alan. "How many men have a brilliant doctor for a son?" He admits that he's always treated his only son like an embarrassment. He figured he'd be able to make amends on his own death bed and tell Alan how proud that he's made him. It never occurred to him that Alan might die first. Tracy tells Edward that Alan will survive if only to spite her. She contends that Alan will never let her have their father's undivided attention. Edward admits that he should've listened to Lila all those years ago and spent more time with their children while they were young. Behind the surgery door, Dr. Ford asks Monica if she was aware that her husband had heart disease. Monica says that his cardiac tests always come back fine. "Your patient's heart is failing," he tells her.
"4 hours. 1 a.m."
"Oh my God, it's Alan!" Monica finally realizes that the man who has fallen to the ground in the midst of a gunfight is her husband. The police restrain her as the chaos goes on. Inside Emily is inconsolable as she begs Mr. Craig to stop the shooting. Upstairs, Luke and Lucky are yelling out the window that Alan is having a heart attack, but the shooting continues. Luke grabs a chair, throws it through the window and out they go to drag Alan behind the police barricade. When the cops see that it's Luke, they hold their fire, but the sniper doesn't. Two swat team members go in and drag Alan to a gurney. Monica can't find a pulse. The paramedics are able to shock Alan's heart into a rhythm and they load him in the ambulance and take off for GH.
At the Q's, Tracey, Alice, Edward and Dillon see that the hostage that has been released is Alan and he looks wounded. They all head to the hospital and are waiting in the GH lobby when he is brought in. Edward whispers in his son's ear that there is only room for one heart patient in this family. Skye tries to say something to Alan, but Monica calls her a bitch and tells her to get away from her husband. Then she explains to Edward and Tracy that if it weren't for Lorenzo Alcazar they would all be safe at home right now. Edward asks Skye if it's true but she runs off.
In the OR, Monica scrubs in to perform Alan's angioplasty and Dr. Ford objects. She pulls rank on him and says that she will be the one to operate on her husband. He tries to reason with her that she's too close to the situation to be objective in a crisis. She tells him to file a complaint if he needs to, but stop wasting her time. In the waiting room, the rest of the family quietly thinks about Alan. Especially Tracy. She tells Dillon that Skye will pay if her brother loses his life tonight. Edward vows to go after Alcazar.
In the lobby, Emily is trying to stall for time in choosing between Alan and Liz. She asks if she can examine each, and at first, Craig refuses. After the female goon suggests it would be more entertaining to build the suspense, he agrees. Emily takes Alan's pulse and tells him that he needs the proper meds to limit the damage from the heart attack. He reminds his daughter that Liz can't give birth this early. He's clearly in pain, but pulls himself together enough to exchange loving conversation with Emily about who is the better doctor between them. Carly sits with Alan as Emily goes to examine Liz. Liz isn't able to speak very well, so Sam tries to explain how frequent the contractions are. When she can, Liz says her contractions are probably due to dehydration. She wants Emily to choose her father.
All eyes in the lobby are on Emily as she finally says, "Elizabeth…I'm so sorry." Craig wants Alan to walk out on his own, so Alan painfully tries to sit up, but says he can't do it. Emily screams to have someone help him, and Craig admires her passion. He reluctantly orders goons #4 and #5 to help Alan to the door. But first, in order to prevent anyone else from trying to flee when the door opens, he puts a on sniper lookout from the second floor window.
Outside, Mac alerts his team that there's movement at the second floor window. Someone asks if it's a hostage. Someone yells that it's a gunman. "Get ready to take him out," Mac commands.
Then, the goons push Alan out the door and into a courtyard. The sniper starts to shoot, and the cops return fire. (As Alan stands out in the open, the footage goes to black and white and in slow motion.) He reacts to sound of gunfire by clutching his chest and falling to the ground.
Nicolas and Emily are tending to Alan in the room down the hall. Alan asks Emily if she appreciates irony. He good naturedly tells her that all these years he worried that Jason would be injured because of his dangerous life and here he is close to death from attending a charity event. Nikolas has found an aspirin and they give it to Alan. Emily whispers that if they don't get a defibrillator soon, Alan will die. Craig comes in and says "Not dead yet?" He makes them all leave – including Alan – so he can be alone with Sam. Craig tells her the only reason he had not killed her yet is because of her "decorative qualities." He unzips her jacket and she calls him a pig. He tells her that he's not a rapist. He asks "sweet Sam" who she cares about in this mess. She tells him to go to hell. He tells Sam that she's totally missing the maternal, nurturing instinct like Liz has. He admires that in her.
Everyone but Sam & Mr. Craig (and Maxi) is now in the lobby again. Carly says there is a defibrillator behind the desk and Sonny makes a good case for the goons to allow them to use in on Alan. Finally, Emily is able to shock Alan's heart into a more normal rhythm. Craig brings Sam back and is angry that his goon gave the defibrillator to Alan. He decides to switch partners again because they're getting far too comfortable. As he announces who moves to sit with whom, there's a sound of a helicopter on the roof and he gets furious. "Bloody hell, somebody just made a fatal error!" he shouts. He grabs Liz by the arm and drags her to the desk phone to call Rica. He orders Rica to remove the helicopter or the lovely Liz will be shot baby and all.
Inside the Metro Court, Nikolas is taken to sit with Alan. Before Craig dumps him there, he suggests that perhaps Nikolas tell Alan what he does with his daughter behind closed doors. Nikolas covers Alan with a blanket and Alan tells him to treat Emily with the respect that she deserves. He tells him that Emily has jumped back into their relationship with both feet and Nik hasn't responded with any kind of commitment. Then, Alan says he can't breathe…and stops talking. Nik pounds on the door for help. They bring Emily in to see Alan. She learns that his pulse is all over the place and he needs a defibrillator. Nikolas appeals to the goon to get him one; but he says that will be just one less person to watch.
