John gets a visit from his former self, Marlena hallucinates her dead sister and Stefano and Rolf scheme.

While John tries to fix the ventilation system, Anna and Tony pass out from the vapor. John fixes things and the vapor is sucked back through the vent, but we hear Stefano's voice over the PA system. He tells John he's too late. John checks for a pulse. Anna and Tony are still alive. Stefano laughs. "Not for long." John rages.

Marlena wakes up and sees her dead twin sister, Samantha. "It's me. Have you missed me?" Marlena thinks she's dreaming. The shot shows she's hallucinating. Sam tells her sister 'Marnie' that she did a bad thing, and now she's going to pay for it! She broke her oath and deliberately hurt another. Sam says Marlena summoned her, herself and she's there to help. She asks why Marlena injected Stefano with the drug. They play a game of back and forth. Marlena tells Sam that someone injected her with the same drug. She begs her to give her the drug that will ** the effects. Sam plays games, and forces Marlena to talk about how she drugged Stefano. Marlena maintains it was not about revenge, but Sam doesn't believe her. She again, asks why Marlena injected Stefano. Marlena becomes agitated, and admits, "Because the bastard deserved it!" Sam cheers! "We're getting somewhere!" Sam comments that revenge was never her style. They flash back to Sam drugging Marlena, when they were younger. Sam tells Marlena she's in no danger but will have a nice long nap. She needs to call Dr. Powell and make arrangements. Marlena's confused. He's a psychiatrist at the Bay view Sanitarium. Marlena tells Sam she was not very nice back then. Sam was misguided, but is making up for it now. She says Marlena has lost her way, because she's so worried about John. Sam comments that the love may have cost her life. Marlena remembers finding John alive, and rehashes when she found out that he had amnesia. Sam's sorry. Marlena thinks it's worse that John has become just like Stefano. She needs to get better so she can help John, but Sam reminds her if he doesn't want help, she can't help him. They discuss John and Belle and how wonderful of a father John is. Marlena flashes back to a Christmas past. John gives Marlena a charm bracelet from he and Brady. She flashes back to John recreating their wedding night, later, and Sam can see how she loved him, but he's gone and Marnie needs to accept that. "I can't do that," Marlena says. Sam remembers that Marlena wouldn't give up on her, either. Sam finds it hard to believe, and they recall the day Sam was sent away. Sam tells Marlena she's glad they're locking her away, but Marlena points out that the doctors are going to help her, once the drugs are out of her system. Sam was able to forgive herself and get better because of Marlena's strength, but Marlena argues that she should thank herself for this. John's in a different place, says Marlena, and he needs her help. Sam finally agrees. Marlena cries and thanks her sister, who needs to leave. They say a tearful goodbye. "I'll see ya, Sis." Marlena needs her help. She asks her to give her the antidote, but Sam can't do it. Only Marlena can. Sam disappears, and Marlena says she can't do it. She tries to move her hands, and inches toward the syringe! She picks it up and injects her leg!

John rushes into Stefano's room and finds a boy with an Italian accent. "My name is Stefano DiMera," he says. John thinks this is a set up, and then tells the kid that they're half brothers. "Colleen ruined my life," spews young Stefano, telling John that Santo left his mother because of Colleen. John says Santo returned, but young Stefano the kid says his pa was angry and things were never the same. John thinks it was Santo's fault, not Colleen's. Young Stefano runs off, yelling, "No!" John goes after him but bumps into Santo, who says, "It's me, Papa." John is angry. Santo abandoned Colleen. Santo says Colleen never told him she was pregnant. He would have loved him, but John wouldn't have wanted to turn out as twisted as Stefano. Santo calls young Stefano as pure as the driven snow, and says he loved John's mother. Santo came to give John a message. They discuss how Stefano stole his memories, but it shouldn't make him cold or unfeeling. John finds this funny that Santo's giving him advice, but Santo hears John was a good man, at one time. He asks that John not become like him or Stefano. He's Colleen's son - a good man. John yells that neither of them have an idea of who he is. He was abandoned! We see John yelling at nobody. He becomes emotional. Santo yells that Colleen did what was best, and Marlena can help him. "She's a good woman, Johnny, and she loves you." John doesn't want to be the man he once was. John hears a familiar voice, telling him that this is not true. "And that's a fact." It's old John! (The hair is the same.) He doesn't like the way new John's living his life. He'd have never moved into his enemy's house. New John thinks they're entitled, but old John says they're already rich. They don't need the money, and it has brought nothing good to their life. New John calls old John a coward. Marlena's not going to go away. Life is about love, family and friendship, old John says. Santo stands by, listening. New John tells old John to get lost, but Santo agrees with old John. Marlena loves him and they believe she can help bring him back. New John's content with who he is, he argues, and isn't sure what he's going to do with Marlena. Santo asks John not to give up on love. He hugs new John, and new John smiles, with tears streaming down his face.

Stefano holds baby Joey and fawns over him. "You're a fine young boy." He thinks Joe's parents have big plans for him, but he has bigger ones. Rolf tells him he has completed what needed to be done. He's happy to be working for him again. Stefano commends him for his help, and both assume nobody knows that Rolf's in Stefano's employ. Rolf takes little Joey and says he'll follow Stefano's instructions to the letter. Stefano calls someone and tells them to wait for instruction. Rolf returns. "It went exactly like you planned."

Tony and Anna regain consciousness, in the utility room. They were hallucinating before they passed out, they realize, and Tony says John must have deactivated the ventilation system. They're breathing easy. They need to find Marlena, Tony says. Anna cries, "How could Stefano do this to us?" Tony will make sure he gets what he deserves.
