Theo was born to Abe and Lexi Carver. He was seen sporadically over the years playing with his parents or grandmother, Celeste. Recently, in fall of 2008, Theo was diagnosed with Autism, and has been seeing a therapist. He seems to have connected with Chelsea Brady, who also works with Theo and the therapist, at University Hospital.
Theo was in the park, in the care of Lacey Hanson, a babysitter, who was gossiping with friends on the phone while Theo was playing nearby. Theo wandered off to see Chelsea at the nearby Hospital, and Lexi and Abe fired Lacey, for not looking after him.
Theo continues to work with Chelsea and Mark (therapist) to help deal with his Autism.

At the mansion, Lexi worries about Theo, when she can’t find him. She sees him on the stairwell, reaching for a painting and yells, “Theo, no!” Theo falls down the stairs and Stefano and Lexi rush to his side. He’s unconscious.
Back at the mansion, a sobbing Lexi asks Stefano to call 911. She thinks Theo may have a skull fracture. Stefano calls the police and is confused when they tell him they’re already on their way. He comforts Lexi, saying Theo will be fine.
Hope and Bo arrive at the DiMera mansion and ask how Theo is. Lexi asks how they knew and rushes off with the paramedics. Stefano wishes they hadn’t let Theo out of their sight but Hope calls it an accident.
Theo, Stefano, Lexi, Bo and Hope arrive at the hospital. Theo has stopped breathing, so Lexi screams while they take him away. Bo and Hope recall being here for Zach. Tony arrives. He just received Lexi’s message. Stefano wants to call Abe but Lexi knows Abe will blame Stefano. Tony offers to take care of contacting Abe but Lexi clings to him, in tears. She worries this will affect the rest of Theo’s life and blames herself. Tony reassures her that she wasn’t negligent but she won’t listen.
Stefano wonders how Bo knew about the accident. Hope refuses to answer and Stefano explains how grateful he is. Later, Tony tells Lexi he can’t get a hold of Abe. Hope comforts Lexi.
Nearby, Hope comforts an angry Bo, who doesn’t understand the use of the visions if he can’t stop something from happening. Daniel returns with news for the family.
Daniel arrives at the hospital and Lexi sobs to him to help her. He goes off with Theo and promises to return with news. Theo turns to Tony for support.
Daniel explains Theo needs brain surgery to reduce swelling on the brain, but Stefano wants to fly in a specialist. Daniel explains there is no time.
Lexi arrives at the hospital with Theo and tells Rob, the therapist, that Theo’s on a gluten free diet and she has adjusted his vitamins and medications. Rob takes Theo into a room and Lexi takes a call from Stefano, asking to give a Christmas gift to Theo. Lexi refuses. Stefano asks her not to destroy their love. Rob brings Theo to Lexi and says he seems agitated. Lexi thinks it’s his food change and they discuss giving him a special reward. She considers this.
Stefano takes Theo into the great room and offers him a Christmas cookie, but Lexi has special ones for him. Stefano doesn’t know what’s going on but Lexi chooses not to tell him. Instead, she just says she knows better for her son. Later, Stefano says he loves when she’s feisty. Just not with him! Lexi’s the boss, she says, so Stefano agrees to go along with her ground rules if he can visit Theo. Meanwhile, Theo wanders off.
Bo starts having a vision of Theo as we hear Stefano and Lexi discuss him. Lexi finds Theo trying to touch a painting. As she yells, “No,” Bo grabs his head from in his office as if in pain.
