In Stefano's room, Ana and Sami start coughing. They hear another news report. They're aware the phone lines and cell phone lines are not in use and report that Salem PD has arrived on the scene. Anna asks Tony what the gas is. John's idea is to find Stefano. Tony thinks that there may be toxins in the gas that will induce hallucinations or worse. They hear something in the vents.
Anna and Tony discuss how Stefano likes to treat his enemies like rats and put them in a little maze and watch them squirm as they fight to live. Tony feels he should have known this would happen. He gets down on himself and says he should have had more strength in character, but Anna disagrees. He has great strength of character and tells him nothing can stop Stefano, once his mind is made up. Tony kisses Anna and he promises they'll get out of there. Anna calls Stefano the devil and wonders how they'll get out of this. Tony asks her not to be intimidated by Stefano. Tony wants to find John, but Anna needs to rest. She's close to relaxing. Tony worries about leaving her alone, but she forces him to go find John.
Chloe finds Lucas and she yells at him to look at her face. "It's horrible." Lucas tells her there is nothing wrong with her face. She's hallucinating. Lucas realizes the smoky substance is a hallucinogenic. "It's probably Stefano," he says. Chloe nods. It was. She saw him outside of Marlena's room. Tony and John arrive just as she's telling this to Lucas, and they go off to Marlena's room. Lucas isn't seeing things, though he has a headache. He doesn't think Stefano's trying to kill them, but Chloe wonders. Lucas thinks Stefano has a score to settle with someone. Perhaps Dr. Evans? Chloe reminds Lucas that he shot EJ. Lucas starts to feel strange and lies down. He passes out and Chloe yells for him to wake up.
John and Tony come out of Marlena's room. John shares that he feels he must have immunity for this gas. He starts feeling the wall, and tells Tony there is a way to ventilate the building. They go off together.
John decides to pay Stefano a visit. EJ and Sami argue about whether or not Marlena should have put Stefano in a paralytic state. EJ thinks she did it to get back at Stefano, but Sami suggests she did it to protect family.
Inside Stefano's room, Anna tells Stefano she wishes he'd never come out of the coma. Stefano calls her a meddlesome witch and Tony asks Stefano to respect his wife. Stefano's not happy to hear they're married. Tony yells that he wants answers and John arrives. He asks what Stefano did to Blondie, but Stefano feigns innocence and John warns him. He'll get another pillow! Stefano thinks John should have done it while he had a chance. Tony and Anna leave, and John asks Stefano if he enjoyed his coma. Stefano says he was fully alert. John already knows this and grins and tells him he took everything he had. Stefano assures him it's about to change. Stefano will reclaim his life. Things are already in motion, he warns. They discuss Colleen Brady, and that Stefano programmed John to kill her, but he didn't do it. Stefano knows and was disappointed. He calls John his pawn. John disagrees. "Right now, I'm the man who can snap your neck like a pencil." Stefano calls Colleen a tramp and John grins devilishly and asks Stefano to call her that once more. John says she died with friends and family around. She's safe from Stefano. "All those that are alive aren't safe," Stefano says. John was told Stefano was once in love with her and asks why he's punishing her.
Outside Stefano's room, Tony and Anna discuss trying to find out what Stefano did to Marlena. Tony worries Stefano won't be satisfied until he's dead. Anna's jaw drops.
John arrives at Marlena's room and tells her he'll help her. Marlena smiles as tears roll down her cheeks.
Later, Tony visits Marlena and is shocked to learn she put Stefano in that paralytic state. Tony knows it was easy to justify, but it was evil. Marlena had to protect her family, she cries. She sounds just like Stefano, and should have known better. Marlena's not sorry but Tony's disappointed. He knows she has a morale code that Stefano doesn't have and he worries that Stefano will try to kill not just her, but all of them!
Sami returns from the roof. It's locked! She's disturbed. EJ tries to make a call, but the phones aren't working. Sami starts to panic. They go off to check the doors and the elevators and meet back at Stefano's room. Since the whole place is in lock down, they assume it's his doing.
Anna returns to Stefano's room, but Stefano is gone! John and Tony arrive and no one knows where he has gone. John says Stefano was faking still being paralyzed. EJ and Sami rush in the door and tell everyone that they can't use cell phones, hospital phones, elevators and the doors are all locked. The television turns on. Itself. A reporter says that the seventh floor of University Hospital is in lock down. It's a quarantine situation! They agree Stefano is behind this and they're trapped here for a reason. Sami notices the gas seeping in.
In Stefano's room, Tony doesn't believe that Marlena could have harmed Stefano. "This woman, this she-devil is nothing but a well constructed façade," Stefano complains. EJ tells Stefano they're just trying to comprehend so they can help. They can't, Stefano says. He rages that he was trapped like a rat. He couldn't move or talk. He's going to repay Marlena in kind. Sami arrives and can't believe Stefano's really awake. She wants to know what he did with her mother. Stefano is not happy to see that she and EJ are no longer married. Sami yells that she won't let him get away with whatever he is planning.
Inside Stefano's room, Tony yells at Stefano for harming Marlena, an innocent woman. He demands to know what Stefano did, but Stefano yells at him for interrogating him. He feels insulted but Tony's not deterred. He continues his rant. Stefano asks where his loyalty is. It's to himself and Anna, points out Tony in anger. Stefano tells Tony that Marlena tortured him, injected him with drugs that induced catatonia. "I was trapped in my own body." He's going to make Marlena pay for the rest of her life, he growls. Tony doesn't think Marlena is capable. Is there proof? Stefano is livid. "My word is not sufficient?!" Tony doesn't know what to think. With tears in his eyes, Stefano explains that he heard everything that was said around him and it was hell. Tony wonders if he had an adverse reaction to the truth serum, but Stefano, through a fit of coughing, denies this. Tony thinks Stefano should feel blessed that he's awake and asks him to think about his legacy. "That you are the bigger man." He wants him to take the high road for once, but Stefano claims that's too late.
Inside Stefano's room, Tony spots Anna, outside the room, crying. He rushes to her and she tells him what happened. She thinks this is Stefano's doing. She's angry with Tony for keeping quiet about Stefano being awake. Her hands go to her hair and she asks if she has helmet hair. Tony says she looks great and admires her nerve and kindness. Anna tells him she is proud of him and his strength, they kiss, and Tony says he has to see Stefano.
