Paul and Dusty face off in the lighthouse, Jack and Carly are attacked, Emily decides to have her tubes tied, Alison finds out she may not be able to have children and Aaron tries to cheer Allie up.
At home, Alison tells Aaron about her doctor's appointment; he says they should meet back at the house afterward to try again for a baby.
Emily meets Susan for breakfast and tells her about her plans to have her tubes tied! Susan can't believe it and asks what happens when Emily decides she wants children. Emily says that isn't going to happen. Susan agrees to be there for her after the surgery.
Alison goes to the doctor's office to be checked out. Chris sees her and asks what is going on but Alison won't talk to him. The doctor runs a few tests and when she returns, says there could be problems. She says Alison has Pelvic Inflammatory Disease and it could make pregnancy impossible! Allie can't believe it and says she feels fine. The doctor says sometimes there are no symptoms but antibiotics can help. She says people who have had multiple sexual partners are at highest risk. Allie thinks about her porn career and starts to cry. She hurries out. Chris sees her crying, hugs her and leads her away. Aaron arrives and finds the doctor. He says he is worried about Allie because she hasn't been home. She tells him about Allie's diagnosis and that they will have problems conceiving. Aaron asks what he can do. The doctor tells him to show Alison that this doesn't change things for them.
Chris takes Alison to Java. She tells him about the diagnosis. He says her past doesn't matter and she needs to move on. She says this isn't something she - or Aaron - can move on from! She can't imagine not being a mother to Aaron's children and breaks down. Chris says she can be a mom, just like she has dreamed, because she deserves it. They walk back to the hospital and see Aaron. He hugs Alison and tells Chris to get away from them. He gets even more mad when Alison says she told Chris everything. Aaron grabs her and leads her away.
Chris tracks down Alison's doctor and questions her but she won't tell him anything.
Aaron takes Alison home and tries to talk to her about their other options but Alison isn't ready to talk. She blames herself and her past and even worries that she may have given him some disease. He says he doesn't care about the diagnosis. Alison turns away from him.
Carly continues CPR until Jack splutters up some lake water. She says this whole thing was a set-up and that Meg is on another island. Jack is bleeding badly from a hand wound but says they have to go now.
Jack and Carly finally arrive at the right island and find the cottage. They are investigating when Jack realizes something is off. He tells Carly to stay there and goes to another room. James hits him and knocks him out. Carly screams and a thug knocks her out! They put Jack and Carly on a rowboat and set it adrift! Carly wakes first. She tries to figure out where they are but can't find any landmarks. Jack tries to rest. He is still losing a lot of blood from the wound in his hand. Carly uses her bandanna to staunch the flow of blood. Jack says she has always made his life better. He gets emotional and apologizes for being so judgmental about her actions. Tired, Jack closes his eyes. Carly sees land and wakes him up.
Paul cocks the trigger of his gun and tells Dusty to get up! He accuses Meg and Dusty of having an affair and he refuses to believe their explanations. He says he saw them on the computer; Meg says he is wrong but Paul won't listen. Meg grabs Paul's gun-hand and says she belongs to him and Dusty is just a friend who was helping her get home. James approaches but no one hears him. Paul puts his gun away and Dusty takes his hand off of his gun. James walks up and tells Paul that Meg is lying! Paul finally sees the light and stands by Meg and Dusty. Dusty pulls a gun and points it at James' head. He offers to kill James for Paul! "It will be a pleasure!" Dusty exclaims. Paul hesitates and then lowers his gun. Meg hugs him. James pushes Paul's buttons and he raises the gun and points it at Dusty. James grabs for Dusty's gun but Dusty fights him for it. A shot rings out as James loses his balance and falls from the top of the lighthouse! Paul runs off. Meg sees that Dusty was shot and helps him down the steps.
Paul races to James' side; he can't believe that James finally called him 'son'. "I was wrong," James croaks and loses consciousness! Paul grabs him and shakes him, begging James to tell him what he did all of this for. "Don't you die without telling me!" Paul yells but James doesn't respond. Barbara runs up with two EMT's and tells Paul to let James go. The medics are unable to get a pulse; Barbara pulls Paul away and asks about Meg. Meg comes out of the lighthouse with Dusty, shocking Babs. Meg goes to Paul, tells him how much she loves him and thanks him for finding them. An EMT takes her to the ambulance so she can be checked out at the hospital. Paul stands over James' body. James' ghost tells Paul that a real Stenbeck would have killed Dusty, not let him leave with his wife. He disappears. The EMT's take James away.
Carly and Jack make it to shore; she contacts Dallas who tells her that Meg is safe but James is dead. She relays the information to Jack, who wants to go to Meg. Carly says he has to take care of himself first.
Dani finds Casey at Java and asks about Emily's surgery; Susan told her about it. Casey tries calling Emily but she doesn't answer. He pays the bill and hurries off to be with her.
Emily meets with her doctor and signs the papers to have her tubes tied. He leaves to get ready for the surgery. Casey barges in and demands to know what is going on. He is shocked when she says she is having her tubes tied. He says she can't do this! Emily says this is her decision and it has nothing to do with him! Casey asks if she loves him; she says yes. He says if she means it, she won't go through with the procedure. He leaves. The doctor returns and Emily says he can go ahead with the procedure.
Casey is alone in Old Town when Dani sees him. She asks about Emily; Casey tells her everything. He says this is another way for Emily to control their relationship and not form any big attachments to him.
