Kendall visits the new doctor; Zach decides to go ahead with the new surgery for Ian. Annie overreacts when Emma is "missing"; Jonathan realizes there is something weird going on with Annie.

Aidan tries to walk away from Greenlee but she won't take no for an answer. Greenlee bugs him until he admits that there has been a "parting of ways". She thinks he is talking about a case and wonders how he can work at a job that allows him to help break up relationships. She offers him another drink when Aidan asks if she has apologized to Kendall. "I've done something better than that," Greens says, but won't tell him exactly what she did. She tries to get him to guess but Aidan won't play along. Instead he tells Greenlee she needs to take a time out from "helping" everyone and instead just leave people alone. Aidan starts going on about "all women" and Greenlee realizes that Aidan's bad mood is because Di is moving to New York! Greens begins rubbing salt in his wound, perfectly happy that someone else's life is apparently going wrong. Aidan can't believe her. Greenlee begins to go on about all the guys who turned her down in high school using clichés; Aidan commiserates, spouting his own break-up clichés. Aidan walks away, leaving her to pay the check!

Annie is frozen in fear as she listens to "Ring Around the Rosie" coming from the phone. Slowly she picks it up and asks why the person is calling. No one answers and she quickly hangs up. Seeing a picture of Emma she calls the day camp to arrange for an early pick up. But Emma isn't there! Annie becomes really worried when the woman tells her that some man, claiming to be Emma's uncle picked her up. She hangs up when someone knocks on the door. It's Jonathan, bringing Emma back. She freaks out on him but Jonathan calms her down, reminding her of Emma's dentist appointment. Realizing that Annie is very overwrought, Jonathan asks what is going on. Annie tries to cover but Jonathan doesn't buy her explanation and tries to get her to call Ryan. Annie refuses, saying she knows it was crazy but she just got worried because of what happened with Spike and Ian. Jonathan asks, again, what is really happening. He wonders why her immediate response was to call 9-1-1 instead of Ryan or Rachel or even him but Annie blows off his concern. The phone rings and Annie hesitates to answer it; Jonathan moves toward the phone. Annie orders him not to answer it!

Zach asks Joe what they can expect from the brain hemorrhaging and Joe tells him that the new problem is fixable. He asks for the nurse and Kendall but neither can be found. Down the hall, the nurse finds only Ryan in Kendall's room. She explains that they need Kendall now and Ryan leaves to talk with Zach. Worried, Zach tells Ryan about the new doctor and his fears that Kendall went off half-cocked. Joe leaves and returns to tell them Kendall's car isn't in the lot! Zach tries calling her but can't get through. When Zach can't remember the doctor's name, Ryan promises to track Kendall down so Zach can concentrate on Ian. He calls Tad to get started. Joe counsels Zach about the problems with Ian, assuring him that if they can relieve the pressure they can help Ian, but they have to do the surgery now. Zach signs off, hoping that he is doing the right thing. Josh walks up and takes the brunt of Zach's anger. He is so mad that Kendall walked out of the hospital with Spike, without telling anyone! Josh sticks around, trying to comfort Zach with what he knows of premies. Calmer, Zach watches Ian, hoping against hope that Ian will come out of this. He watches as the docs wheel Ian down to surgery. "Kendall has no idea how much she was needed here today," he says.

Ryan and Tad meet in Kendall's room and begin going through her things to find out where Kendall may have gone. Tad tells Ryan to calm down because the doctor is probably really trying to help but Ryan still worries that there could be problems because Kendall hasn't been sleeping. Tad gets a guy at the phone company to trace all of her calls but that isn't enough for Ryan - he wants to find Kendall now before anything else happens to Spike. Tad tries to calm him down, reminding Ryan that Kendall loves Spike and she wouldn't do anything to hurt him. Ryan calls Annie but can't make sense of anything she says. He asks her to think about any other doctors Kendall may have seen but she doesn't know. Ryan hangs up and learns that Kendall may have seen a Dr. Hilliard. Ryan takes off to find them.
Annie hangs up from talking to Ryan, more worried than ever. Jonathan goes to help Emma with cookies and she makes a call. She learns that whoever she was calling for hasn't left wherever they are as Jonathan and Emma return with a plate of cookies. He takes and few and leaves; Annie locks up behind him. She picks up Emma, holding her close because she is still worried about the weird calls.

Kendall is at the new doctor's, looking at the pictures of the kids who have been helped. She talks to an adult patient who encourages her that the new doctor can help Spike. The doctor calls her in to his office. Her cell phone rings but the doc asks her to turn it off; she does and they begin talking about other options for Spike. She asks about cochlear implants and the doctor tells her that should be the last resort. He gives her some literature from his patients and Kendall begins to check him out; Dr. Hilliard takes Spike to begin his examination. He first recommends herbal vitamins and other natural supplements and tells her that they will go on from there once all of the surgical toxins are out of Spike's system. Spike jabbers, grabbing envelopes and looking at the book as the adults talk. Kendall tells him she'll go along with the new treatment. She puts Spike down to play and discusses the treatment options, telling Dr. Hilliard about Ian, too, worried that Zach may object to the new treatment possibilities with Spike. He explains that she has to get Zach on board, otherwise the treatment won't work. "You have to focus on this child," he says, basically telling her that she can't have Ian's problems on her plate, too. She swears that Spike will be her only priority and they go on to talk about organic treatments. Kendall tries to pay him for the visit but he refuses, telling her that he treats all children regardless of their ability to pay. He tells Kendall to check Spike out of the hospital and they'll begin twice weekly treatments immediately. Just then Ryan walks in!
